Parents, if you will have a child playing high school sports in Houston ISD, please adhere to the steps below. 

1. Begin by filling out all required forms online. https://www.registermyathlete.com/login/

2. Submit your Insurance coverage information in case of injury.

3. Complete a Physical Exam with a medical doctor. See below for form.

4. Communicate with the Coach for additional steps.
